Required Documents for New Student Enrollment

 

The Following documents are for NEW student enrollment. All documents must be submitted to the office IN PERSON upon completing online enrollment.

Proof of Birth Date

  • Birth Certificate
  • Statement by local registrar or county recorder certifying date of birth (valid for up to 30 days from enrollment)

One Parent/Guardian Photo Identification

  • Driver's License
  • Identification Card
  • Passport

Address Verification (three total)

  • Current PG&E Bill, including parent/guardian name and address
  • Current Water/Utility Bill, including parent/guardian name and address
  • Mortgage Statement or Lease Agreement, including parent/guardian name and address
  • Bank Statement
  • Mail (with current date)

Proof of Immunizations 

  • Child's Immunization Records (see links below for required immunizations)

Required Immunizations (English)

Additional Documents

  • If applicable, Special Education (Individualized Education Plan; IEP/504), Legal/Custody Documents, and/or Health Documents.
